If your HSA/FSA administrator requires a specific format, we can create one for you. Simply email [email protected] with:

• Your order number

• The exact details or wording your administrator requires

⚠️ Limits: We cannot add diagnosis codes or healthcare provider information.

Alternatively, you can always download a receipt directly in your patient portal by clicking Receipt next to your order. You’ll have two formats to choose from:

• Detailed receipt – shows all test names

• Private receipt – no test names shown
