How will I receive my requisition papers for testing?

You will receive a notification email informing you that your requisition papers are available. The email will be called "Requisition Papers Available".

You can download the requisition in this email by clicking on the attachment at the bottom of the email.

The email will also contain a link to the secure Patient Portal where you can read the communication accompanying your requisition. We highly recommend reading it as it contains important information about the tests you ordered, and it provides answers to many frequently asked questions.

Please read the email for important information associated with the login process. 

Once you have successfully logged in to the secure Patient Portal additional instructions will be available to guide you to the appropriate area to read the special instructions. 

You can also find your requisition by logging into your secure portal at https://patient.privatemdlabs.com/login and downloading it from your orders tab.
