Log into your patient portal
Find the order you need
Click Receipt next to that order
Choose one:
Detailed receipt – shows all test names (often required by some HSA/FSA admin)
Private receipt – no test names shown
Download and submit to your HSA/FSA administrator for reimbursement
Need a specific format?
If your HSA/FSA administrator requires custom wording or fields, email [email protected] with your order number and the exact details they need.
(Limits: we can’t add diagnosis codes or healthcare provider information.)
