You should not have this test done within one week of receiving a vitamin B12 injection, as recent B12 therapy can interfere with the accuracy of the results. Other than avoiding B12 injections for a week before testing, no special preparation is typically needed. You can take your regular medications and eat normally before this test.
This test measures Intrinsic Factor Blocking Antibody, which detects whether your immune system is attacking intrinsic factor, the protein your stomach makes to help absorb vitamin B12. When these antibodies are present, they block B12 absorption in your intestines, leading to deficiency even if you're eating B12-rich foods or taking oral supplements. This is the most specific test for diagnosing pernicious anemia, an autoimmune condition that requires lifelong B12 management.
