Once you have decided to place an anonymous lab order with Private MD Lab Services, all that remains is paying for the test(s) without disclosing your identity. You can do this in one of three ways:

Purchase a Gift card
Make a cash deposit into Private MD Labs' Bank of America account
Pay by Money Order or Cashier’s Check

Gift Cards

You may purchase American Express, Discover, MasterCard and/or Visa (EXCEPT Vanilla Visa sold at CVS) Gift Cards from a retail store to cover the amount of your purchase. Many times multiple cards must be purchased due to the availability of the needed dollar amount. Normally $25, $50 and $100 gift cards are available at most Grocery, Pharmacy and Department (Wal-Mart, Target) Stores. Larger domination gift cards are produced, but not always readily available. Please be aware when purchasing the gift cards, the retailer will charge an activation fee per card, which will vary depending on the domination of the gift card being purchased.

Please note: We have experienced difficulty in processing transactions using Vanilla Visa Gift Cards. The Vanilla Visa Gift Cards are sold at CVS Drug Stores. The transactions are declined when using this type card for reasons that cannot be explained when contacting Vanilla Visa directly. We have only encountered issues with Vanilla Visa Gift Cards and suggest purchasing a different type of gift card for use on our site.

Because gift cards are not associated with an address, orders using gift cards as the form of payment must be processed via telephone, please call our office 877-283-7882 during normal business hours (M-F, 8:00 AM – 4:30 PM Eastern Time) to place your order using a gift card.

How to make a cash deposit into Private MD Labs' Bank of America account

Once you have chosen the test(s) you would like to order, calculate the prices of the test. There are no hidden fees; the price listed on the site is the amount you will need to deposit in CASH into our account.

Go to any Bank of America with the exact amount of CASH for your order. Ask for an out of state deposit slip, check Tennessee for the state of the account.
Enter the following information on the deposit slip:

            Business Name: Private MD Labs
            Address: Dickson, TN 37055
            Bank of America Account Number: 4440 0673 3513

Enter the amount of your deposit in the area for cash on the deposit slip and the total deposit on the total line.
Give the cash and the deposit slip to the bank teller and wait on your receipt. Keep the receipt for your records.
Contact our office 877-283-7882 to notify our staff of your deposit. You will be asked for the following information:

Name (please remember this alias) to place on the lab requisition, which is your doctor's order
Date of Birth
Gender
Phone number to reach you in case of technical issues with email
The lab location that is most convenient for you
The test(s) that you would like to order
The amount of CASH that you deposited into our Bank of America account
The email address where you wish to receive your requisition and results

Once the deposit has been confirmed, your order will be processed and you will receive your requisition (doctor’s order) and any special instructions related to your test(s) via secure email for you to print and take to the lab.

If you do not have access to a printer, contact our office again at 877-283-7882 so that we can fax the requisition to the chosen lab.

How to pay by Money Order or Cashier’s Check

Once you have chosen the test(s) you would like to order, calculate the prices of the test. There are no hidden fees; the price listed on the site is the amount you will need to get a cashier’s check or money order made out for.

Send a cashier’s check, money order or other certified funds (NO PERSONAL CHECKS) along with the following information to our address listed below:

Name to be listed (please remember this name) on the lab requisition, which is your doctor's order.
Date of Birth
Gender
Phone number to reach you in case of technical issues with email
The lab location that is most convenient for you
The test(s) that you would like to order
The email address where you wish to receive your requisition and results

We suggest you send a money order or cashier's check in a traceable package, such as UPS, FedEx or delivery confirmation via US Postal Service.

Send the money order, cashier's check or certified funds to:

Once the cashier's check or money order has been received, your order will be processed and you will receive your requisition (doctor’s order) and any special instructions related to your test(s) via secure email for you to print and take to the lab.
