Welcome to the USANA 2010 Vitamin D Study

You have been selected to participate in the USANA 2010 Vitamin D Study. You will have 7 business days to complete the blood draw. After seven days the requisition form will expire. The blood sample will be collected at a LabCorp Patient Service Center of the participant’s choosing. No appointment is neccessary.

Private MD Labs is the official lab testing partner of USANA for the Vitamin D Study. Enrollment in the testing phase of the program is easy:

1. Find and select a lab near your home or workplace
2. Fill out a quick form with your name and contact information
3. Your lab requisition and test results will be sent to you electronically. You will receive instructions on how to retrieve this information once you have submitted your lab request

NOTE: Please have your USANA Study ID number handy because you will need it to complete the lab registration process.
